Social Worker, MSW, BSW or LPN - Care Coordinator - Sign-On Bonus Available.
The Care Coordinator assesses and determines the level of care and appropriateness of community-based services for prospective clients. The Care Coordinator also reassesses client needs in response to condition changes or requests from service providers or other case management team members, and relays information to physicians and other health care partners. The area of coverage is ROME, GA and neighboring counties. This is not an Atlanta-based position.
Community Care Services Program (CCSP) helps senior adults and people with functional and/or cognitive impairments stay in their homes and communities for as long as possible. CCSP is a program of the Georgia Department of Community Health (DCH). Visiting Nurse Health System provides case management oversight for more than 2,100 CCSP clients in the Greater Atlanta area at any given time, allowing them to access community-based care and retain their independence.

POSITION REQUIREMENTS
MSW candidates must possess a Master's Degree in Social Work, Sociology, Psychology or a related field; or a graduate of an accredited baccalaureate school of nursing, with at least two years' experience in case management in a health care field. RN or SW licensure preferred, but not required. Current CPR certification required.
BSW candidates must possess a Bachelor's Degree in Social Work, Sociology, Psychology or a related field, with at least two years' experience in case management in a health care field. Professional licensure preferred, but not required. Current CPR certification required.
LPN candidates must be a graduate of an accredited Licensed Practical Nursing Program or School of Nursing and have current and valid licensure by the Georgia State Board of Nursing, with at least two years' experience as a licensed professional nurse in the area of community health, long-term care, or geriatric nursing. Experience in long-term care preferred. Current CPR certification required.
ALL candidates must be able to function independently and effectively in a community-based environment. This role requires proven interpersonal skills with the ability to communicate effectively. Candidates must demonstrate proven decision-making skills, organizational, self-discipline, and time management skills. Additionally, all candidates must be proficient with general use and functions of computer technology, such as: user names and password concepts; Internet; e-mail; navigation of computer desktop or laptop, including starting programs, using files and windows, effectively using navigation buttons and tool bars; ability to self-manage online HR services and online training programs. All candidates must have reliable transportation and current/valid vehicle insurance.
